Contractor Information required pre-start date
When starting a new contract we require the following:
All Contractors
- Completed Candidate registration form. Download here
- Copy of Passport
- Proof of Address (we are unable to accept a mobile phone bill)
- Proof/copy of valid Visa for employment – if required.
Umbrella Company Contractors (UCC)
Please let us know which company you are going to work with and then we can co-ordinate the contracts. (E.g.: Bank account details etc. will be sent via the umbrella company to Durham)
Durham Professional Services does not work with off shore Umbrella companies.
Limited Company Contractors (LCC)
For LCC we need to see the following:
- Certificate of Incorporation
- VAT registration Document (if registered)
- Bank Account Details.
- Signed contract
